EVENT TRAILER RESERVATION
Please complete all fields in the following form. Remember, each church may have no more than 2 dates reserved on the calendar at any given time. Any changes to dates or places to be used must be re-submitted online. You will receive confirmation or notice of no confirmation within one business week of our office receiving your request.
By submitting this form, I acknowledge that I accept and agree to the following:
I have read and agree to abide by the policies provided on this website. (required)
I understand that as the person who requests the use of the Outreach Event Trailer, I am the one responsible for its use and its return according to the policies provided. (required)
I, representing the church, voluntarily release, indemnify, hold harmless and discharge the Maine Baptist Association from all liability, claims, demands, actions, or rights of actions, whether personal to me or to a third party which are related to, arise out of, or are in any way connected with my rental of the inflatables, popcorn popper, snow-kone machine and all other equipment included with use of the Outreach Event Trailer and its contents, including those allegedly attributable to negligent acts or omissions. I agree to reimburse any reasonable attorneys fees and costs which may be incurred by the Maine Baptist Association in the defense of any such liability claim, demand, action or right of action. I understand that my church must obtain liability insurance for said event and copy of verification rider sent to MEBA. (required)
I understand that the Outreach Event Trailer fee is $100 per single event day plus $50 for each additional event use during the rental period. (required)
I understand that we are responsible for picking up the Outreach Event Trailer at the MEBA Office in Augusta and returning it to Augusta within 1 day of our event, properly packed with equipment clean. (required)
I understand that failure to return the Outreach Event Trailer with equipment clean and packed properly will result in an additional fee of $75. (required)
